Club Sport Forms

Below is a list of Club Sport Forms. Please refer to the Club Sport Officier and Participant page for details and more information for each form.

Participants who are 18 years of age and older:

Before your first team activity, including tryouts, practices, etc. 

Complete Club Sport Waiver & User Agreement and Code of Conduct online via MyRec.

Participants who are 17 years of age and younger:

Before your first team activity, including tryouts, practices, etc. 

Waiver & User Agreement - To be completed by the Parent/Guardian

Code of Conduct - To be completed by the student participating in the Club Sport

Space Request

Recurring/One-Time Practices and Single Competition

Complete this form at least two weeks before a Single Opponent Home Competition or a Recurring/One-Time Practices and Single Competition

Space Request

Multi Opponent Competition 

Complete this form at least two weeks before a Multi Opponent Home Competition 
Drivers Forms

If a Club Sports Participant wishes to use their personal vehicle or drive a vehicle for travel (away competition or off-site practice) they must complete the university’s Driver’s Aware Class and the Club Sport Approve Driver’s Form.

Step 1: Contact your Club Officier (or Leigh Fitzpatrick) for club specific information required to register for class.  Information you will need Business Office Contact and your team specific COA. If your Club Officier is unsure of this information, please contact  Leigh Fitzpatrick

Step 2: Complete the Driver’s Awareness Class (required before you can fill out a Driver’s Form)
The online class about driving safety is managed through Fleet Management. The online course takes 30-45 minutes to complete. You only need to take this class once every four years.

Step 3: Complete the Club Sport Driver Verification 
This needs to be completed every year before you begin driving.

In-Region Travel Complete this form for any In-Region Travel for competitions or practices. 
Out-of-Region Travel

Complete this for any Out-of-Region Travel.

Unsure of what qualifies as In-Region or Out-of-Region Travel?

Bus Requests Complete this form AFTER you have submitted an In-Region or Out-Of-Region Travel form.
Travel Departure from Campus Complete this report as your travel group departs from campus.
Travel Return to Campus Complete this report as your travel group returns to campus.
Annual Re-Recognition  Each September, each club will need to complete this as part of their re-recognition process.
Active Roster

Complete this form as your add/delete members from your team’s active roster. 

Each team may define their Active Roster differently. Typically an active member is anyone who continues to participate in your club’s activities after tryout, learn-to-play phase, or first few weeks of the semester ends. 

Injury Report

After sustaining an injury or illness during scheduled Yale Club Sport practices or competitions.

The individual completing the injury report should be a Yale Club Sport participant or a Department of Campus Recreation Staff and include only the basic information such as the injured person’s name, contact information, facility at which the injury occurred, location of the injury, and nature of the injury.

Incident Report

To report any incidents, which do not qualify as a injury or illness such as misconduct, facility issues, etc. 

The individual completing the injury report should be a Yale Club Sport participant or a Department of Campus Recreation Staff and include only the basic information such as the injured person’s name, contact information, and a brief description of the incident.
